Tip of the day 💡: This intramedullary spinal cord signal is “normal” on STIR sequences. Don’t call it “myelitis”!

Disappointing negative outcome from the THEIA trial of IV alteplase vs aspirin in CRAO, albeit underpowered. Similar outcomes reported at ESOC 2025 in the yet published TenCRAOS trial of IV tenecteplase. Is intra-arterial thrombolysis the answer here? 🤔 thelancet.com/journals/laneu…

Size Does Matter Wartenberg in 1945- "It is a common mistake to regard the reflex as lost when it is not." If Jendrassik's maneuver can't "find" DTRs, areflexia of GBS, Miller-Fisher, Holmes-Adie may be supported but first try a bigger hammer (not shown) on the muscle tendon!

Hemi-orolingual angioedema is known to occur after thrombolysis for ischaemic stroke (esp. with ACEi), but I was not aware it could also occur in ischaemic stroke WITHOUT thrombolysis 🤯 Possibly linked to infarction involving the insula. ahajournals.org/doi/10.1161/ST…

I love gait analysis! Let me break it down for you. Look at the slow and careful steps with low step length and height with the feet close together and yet so many sudden sways from side to side. This is a strong sign of a non-organic etiology. This is because a wide base is

Although I like to use "amaurosis fugax" for vascular events, "amaurosis fugax dolorosa" is fascinating! A painful (headaches or periocular) transient monocular visual loss which may herald MOGAD optic neuritis. Not positional. Rare (2%), not one to miss! neurology.org/doi/10.1212/NX…

Functional Neurological Disorder is still too often labelled as “medically unexplained”. That’s outdated. Here are 5 reasons why #FND shouldn’t be grouped under #MUS 👇
