The team from Hywel Dda UHB have shared how a project at Bronglais General Hospital has significantly increased accurate completion of fluid balance charts. Their great improvement has sustained and is now spreading across the health board. #SafeCareCollaborative

A project being implemented at Hywel Dda UHB as part of the #SafeCareCollaborative is improving communication regarding palliative and end of life care, to support patients to die in their preferred place of death through having the opportunity to record statements of wishes.
