As an experienced ST6 psychiatrist I am asked to refer to a specialist consultant and team for ADHD. What the cinnamon toast is this nonsense. You can't just rock up and diagnose.

Some recent debate in Parliament about the GP independent contractor model. But let’s be clear. NHS GPs in the UK are not “independent”. They work under a very tightly regulated NHS contract. It’s not a model of independent practice that doctors outside the UK would recognise.

GPs believe they're patients - a key part of the trusting relationship. There are no objective 'markers' (ok, PHQ9, GAD 7 can assess severity).

The Wounded Healer Book Club kicked off with a powerful discussion on Dr Rachel Clarke's book 'Breathtaking'.

Her harrowing account of the Covid-19 pandemic sheds light on healthcare workers' experiences then and now.

Here is one of the key messages she wants readers to take

‘More medical evidence’ ’Perhaps medical’
But Rishi doesn’t want sick notes from GPs

NOT funding GPs (20% cut in 8yrs)

The UK spends very little on things like employment benefits, sickness benefits, pensions compared to peer nations

So if Walmart can’t make healthcare profitable, maybe we should consider medicine differently than retail? 🤔

Oh this is brilliant NHS Employers 👏🏼👌🏼🙌🏼

A huge document on the crucial issue of RETENTION of “NHS People” w/ almost no mention of PAY!

“Improved culture, wellbeing & leadership” mean nothing when the majority of NHS HCPs are paid so poorly & Jnr Drs are treated so badly too.

It's May.

So let's start this month by quoting a bit of Sir Michael Marmot to all of you folks who are trying to tackle 'inequalities'

'People are not poor because they make poor choices.

Poverty leads to poor 'choices' or more precisely, no choices'

'If we focus on sick notes without fixing the underlying problems in access to healthcare that are causing more people to spiral into worklessness, we’ll only create a punitive system for patients who have already been grossly let down.'

This time last year Rishi Sunak met 3 suicide-bereaved dads & assured them better mental health support was a priority for his government. Today he is sending thousands of vulnerable people into despair by threatening to strip them of the (already meagre) benefits they live on

Can we *please* stop telling med students and trainees that financial hardship is part of the journey and 'you'll earn it back'? The landscape has changed entirely (debt higher, specialist pay lower) and there's nothing honourable about equating poverty and 'commitment'. 🤨
