This week’s #SPIN-#POV: Have you ever spotted “dark white matter” on FLAIR images in a patient with seizures? That subtle FLAIR hypointensity can help identify the seizure focus. It’s not pathognomonic and can be seen in a variety of conditions (this was MOGAD).

And the reverse, kind of “doppelgänger”. In this case a cortical tuber: ⬆️FLAIR & hypoperfusion (ASL) ☝️In CET, hyperperfused lesions seem to be associated with a higher frequency of seizures

Do not let an early life normal appearing FLAIR beguile you. The other sequences were telling in this case of Tuberous Sclerosis, the FLAIR glows with the tubers a year later!

Tip of the day 💡: When a child with a known arachnoid cyst presents with worsening headaches, have a low threshold to re-scan, as it may show a spontaneous rupture of the arachnoid cyst with a subdural collection. The before and after presented below.
