Doses of antibiotics are generally optimised to produce optimal bacterial killing effect. However, the effect of resistance emerging to these agents is not just not taken into account; it is typically not even characterised as part of the drug development process. 2/n

This is the fundamental root cause of all antibiotic resistance. We use antibiotics at a dose that kills the bacteria enough to treat an infection, but not to suppress the resistance that emerges (either in the infection or elsewhere in the body) 3/n

This is especially a problem in pneumonia. These infections have a large volume of infective material with large numbers of bacteria, which are high enough that natural rates of genetic mutation provides opportunity for mechanisms of antimicrobial resistance to develop. 4/n

This is particularly true for hospital-acquired pneumonia (HAP), which are caused more often by Gram-negatives (with P. aeruginosa being a common cause), which have a greater propensity to develop resistance on treatment. 5/n
