ESRD is a bitch

Ms K is a 69 yr old pt with ESRD who was doing very well CAPD but had to be shifted to HD d/t refractory peritonitis.
After she asked to be shifted back, she chose APD this time. This is her drawing of the circuit she made during her training.

65M ESRD, HFrEF with EF 15-20% presented to outside facility for Nstemi an aoc CHF. Diffusely diseased vessels, not a CABG candidate. Impella mcs, orbital, prox to mid lad first for outflow, then TAP ostial circ and left main into LAD.

End-stage renal disease (ESRD) = glomerular filtration rate (GFR) less than 15 mL/min/1.73 m² (milliliters of cleansed blood per min per body surface) --> Kidney function severely diminished, requiring renal replacement Rx (dialysis or transplantation) to sustain life

I am extremely painful and associated with poor prognosis

I am seen almost mainly in ESRD patients

I am also known as calcific uremic arteriolopathy

✔️ RTX and CYC are both effective for induction
✔️followed by RTX at least 500mg q6 mo for at least 2Y

For #HAIDI (Hemodialysis access-induced distal ischemia) pts with arteriosclerosis in the forearm arteries, never try to perform ulnar/radial angioplasty. I always use PAI procedure with 4mm end of ePTFE at proximal axillary. Much more reliable than Miller operation. #AVF #ESRD
