For 99% of software teams, improving the reliability of their code would reduce its cost. You have to reach safety-critical levels of reliability before it becomes the tradeoff most developers claim it is. It's always been a skills issue.

XP never had a story about scaling, which limited its reach. I get the feeling that there is a good story to be told about helping 1000s of folks coordinate their efforts & that story starts by reducing team dependencies.
