Is anyone aware of good writing on the topic of RCT design for diagnostic tests?

Have seen the complaint that Cochrane Reviews do not have open data several times. This generally seems like a misunderstanding of what a Cochrane Review is. What data? Reviewers extract summary data from studies, and all of this is included in the review. What's left?
