Full room! Fantastic lecture by George Tolis on post-infarct VSD. ✏️ cardiogenic shock suspecting VSD, make sure to *LISTEN*🩺 before PCI ✏️ IABP (afterload reduction, decrease shunt. NOT impella) ✏️ Surgery vs. ECMO(if in metabolic disarray) 👉surgery w exclusion technique

The only reason for an Impella combined with ECMO is lack of adequate pulsatility. In a post MI VSD, pulsatility is not an issue since the MI is never massive (patients have been walking around with it for a few days). A case report where you got away with it is not an indication

Another rare case : middle mediastinal lipoma abutting carina, esophagus, left atrial dome, SVC and encasing LMCA, requiring transection of aorta and main PA and caudad rotation of the heart for excision. Suckers in pic.3 are in aorta and PA bifurcation.
