In paediatric IBD field we as clinicians stopped including patients in case they could be randomized to placebo because this indeed as unethical. And published a manuscript about this, pubmed.ncbi.nlm.nih.gov/30979718/. This has changed practice. No more placebo in paediatric IBD trials.
