Emergency Medicine. Pragmatism not idealism. Meaningful minimal interventions and only when required. Prefers talking to tubes. Patients first. My views only.

Don’t skimp on your documentation.

Don’t leave it to the end of your shift.

'You can't talk about it without smashing your head against a metaphorical brick wall... and it's self-inflicted.'

Outstanding presentation from ⁦Ian Higginson 🌈⁩ about crowding. The single most important issue facing our specialty. But, with the right policies, this is a fixable problem

The key to effective RAT is getting patients out of RAT quickly to assessment areas, SDEC, primary care or home.

If this doesn’t work, all that’s achieved is duplication and over investigation / diagnosis / treatment.

And it’s knackering.

But actually I don't really hold the single organ / disease teams responsible.

Executive teams are responsible for enforcing accountability. And the evidence seems to be that they avoid doing this. Because it's too hard or unpalatable.

Until they do, we will fail.

Accountability. Of others in the system.

EM is accountable for everything. When other services don’t work / stop working / seem like a “hobby” where is the accountability?

Back pain to end the day - or as I think of it these days, spinal pain.

Handovers are fraught, handovers of spinal pain are notorious in my mind.

Writing a report to the Ombudsman after someone was discharged nearly 3 times in one night with a wry neck will do that…

Really interesting from Gareth Grier on the early management of hyperacute brain injury and impact brain apnoea.

Early bystander management an area we can improve on.
