Our nurse just filled out a prior authorization that had 61 questions & took 59 minutes to complete

For just one medication—and the patient is already on it & doing well

She could have cared for 4-5 patients/families during that time. This shouldn’t be legal

Hey

In middle of great read re systems, &it prompts this:

, esp , is NOT about propaganda, commercials, quarterly returns, efficiency, scale, data analysis, metric achievement

It IS about stories, trust, authenticity, listening, time, team

Sharing this work completed during my time as an NCSP at UCSF health policy/leadership fellow with the support of many, including Urmimala Sarkar, MD, MPH Dawna Ballard, Ph.D. (she/her/hers)

It is time to stop clinging to organizational habits that are unsustainable for our workforce.

“There's something happening here.
What it is ain't exactly clear...”

- Buffalo Springfield, “For What It’s Worth.”

I look at the man standing in the corner. He has been following me. In and out of every exam room.

Wherever I go, he follows.

Who is he? 1/

So many doctors I know are leaving primary care.

They're doing specialty fellowships or leaving medicine entirely, burnt out from the grind.

What keeps me going as a primary care doctor?

1) Working with students. They keep me honest and excited!

Hey

Where are the self preserved ‘leaders’ who’ll hear an impassioned plea borne from lived experience &hard work, who won’t then reply, ‘Thats an emotional argument, show me the data’

Beware data demand, esp from those who are agnostic &see no pts

Hey

Doing some peer support/mentorship on my own time - a leader, built an important clinical program from scratch

2men brought in 2take over, redoing things (??)

When do we learn, AMCs?

It’s the best profession in the world, but sometimes I’m sad for us

I feel like each &every time we renew/prescribe a sulfonylurea in we should be stirred to fight

They are harm, inferior care, disparities, and a reminder of evidence blocked, data inequitably applied &gross pharmaceutical protections

The Annasthesiologist I for one think that we should start sending our patients to the supreme court for medical clearance prior to surgery. It would unload an awful lot of work from the doctors, and the court seems to like making big medical decisions for people, so it would be appropriate.

To my followers who wonder what MOC is, and why many doctors are tweeting about it. Thread.

1) Maintenance of Certification (MOC) is a redundant requirement thrust on US physicians by a private organization. We resent it.

The pursuit of revenue at all costs in is morally bankrupt

Patient &community in center, not revenue in center, will keep mission - the revenue will come, &it will come in a way consistent w oath &values

We’d be far less distressed

I can’t help wondering whether the most radical move of all would be a leader, investor, or institution with the courage to tackle the policies, biases, and structural incentives that are sickening the health care system itself.

There’s a lot I love about ACP’s annual meeting, but without question my favorite part are the constant reminders of how many remarkable people I’ve gotten to know in my career. What an absurd privilege! Hard to lose faith when you’re constantly seeing heroes.

The perfect baseball photo, capturing not just eight stars from a legendary team, but precisely the right eight stars. These were the Boys of Summer. RIP to each and every one.

What do Jackie Robinson and Carly Simon have in common?

In 1952, the Robinsons needed a bigger home after the birth of their 3rd child.

The family were denied homes in NY that were 'mysteriously' pulled off the market. They had trouble viewing any new properties. (1/3)
