There are core features of personality pathology, defined by internal psychological processes. There are peripheral behavioral expressions of these processes (ie, symptoms)

The former are enduring, the latter come & go with circumstances

Most research measures only the latter

Matt Walsh We've learned a few things over the past ~125 years of treating mental/emotional difficulties. At the center of of what we’ve learned is that telling people 'exactly what your problem is” solves NOTHING

Psychological illiteracy on social media has truly reached epic proportions

When I was a kid, I remember seeing an infomercial for some weight loss product that you would strap to your abdomen and it would simulate sit-ups as you just sit there. Kind of reminds me of those promises to treat personality disorders in 8 or 12 or 16 sessions.

The North Dakota State Hospital, Jamestown, North Dakota, circa 1900. Built in 1885, it reached peak population in the 1940s. In 1998, several hospital buildings were converted to a medium-security prison, an ironic symbol of our nation's criminalization of severe mental illness.

2 1990's drivers narrowed psychiatry focus from biopsychosocial model to pills:
1)NIMH 'Decade Of The Brain'
2)Insurance reimbursement cut to 20 minute med visits

Harmful Results:
1)Psychiatists dont know patients well enough
2)Overdiagnosis
3)Overmedication
4))Fragmented care

Little in DSM/ICD is really new- most psych problems described often in past & human nature pretty stable.

No great need to change DSM/ICD labels in future- most recent changes caused more harm than good.

We needed more understanding of causes & better treatments.

Bleuler writing in 1911: 'As a rule, catatonic symptoms mix with the manic and the melancholic conditions, in some instances to such a marked degree that the catatonic symptoms dominate the clinical picture and one can speak of a manic or melancholic catatonia.'

'Catatonia' among most missed/misdiagnosed/mistreated psychiatric symptoms.

Confusion due to too close association with schizophrenia- where it's now rare.

Much more commonly caused by neuro or other illness/bipolar disorder/depression/substances.

Here's very useful resource:

'And so, it is not astonishing that, though the patient enters therapy insisting that he wants to change, more often than not what he really wants is to remain the same and get the therapist to make him feel better.' - Sheldon B. Kopp

Took a phenomenal auditory voyage this week.

First, Mark L. Ruffalo and Jonathan Shedler on Carlat Psychiatry discussing psychodynamic psychotherapy and the term β€œevidence based.”

Then, David Puder M.D. and Judy Beck on CBT.

The episodes pair brilliantly πŸ™Œ
