Just because it’s GDMT, it doesn’t actually help the patient if: they can’t tolerate it, don’t take it, or can’t afford it.

Of course we always aim for GDMT, but sometimes you have to find a compromise with what is also realistic for the patient.

I will never forget writing my advanced directive and designating my then division chief as my DPOA as the pandemic started, in case I needed to be admitted to my own ICU and intubated, etc.
