This is correct. The history of legal protections for patient records, in the US at least, stems from privacy violations of HIV and AIDS patients, particularly gay men for whom the exposure of HIV status doubled as outing.

As I understand it it's not merely the ethical issues of a blind study--it's the impossibility. As someone who spent much of her own childhood trying to will her body into a shape it refused to take, I can assure you there is no placebo effect that will do the trick.
